Four weeks of eating vegan, and something shifts in the blood: the patterns that control which genes the body switches on and off. That is the result of a randomised trial published in early August in the journal MedComm, run by researchers at UC San Diego together with the University of Freiburg. German public broadcasting picked it up this week, and the headlines sound big: against inflammation, against ageing.

We read the study itself. It is more interesting and more careful than the coverage suggests. Three details keep getting lost, and one of them comes from the same research group.

What did the study find?

48 healthy adults ate either a vegan or a meat-rich diet for one month, randomly assigned and matched for calories. Afterwards the vegan group showed different DNA methylation patterns at genes for immune response, metabolism and cell growth. Two of three epigenetic ageing clocks showed a slowdown, the third did not.

Important: what was measured are chemical marks on the DNA, not the activity of the genes themselves. And the widely quoted shift in immune cells was calculated from those marks, not counted. The effects are small and the window is short.

What was actually done

This was a randomised controlled trial, the most reliable design nutrition research has to offer. 48 healthy adults first ate a standardised diet for a week, then were randomly assigned to one of two arms: four weeks vegan or four weeks meat-rich. Both diets delivered the same number of calories, so that the comparison is about composition rather than quantity.

Blood was taken before and after the month, and genome-wide DNA methylation was measured at more than 800,000 positions. Methylation is a chemical mark on the DNA. It does not change the letters of the genome, but it influences how readily a gene can be read. Hence epigenetics: a layer above genetics.

The first author is Lukas Karbacher of the University of California San Diego; the work was done together with the group around Maximilian Storz at Freiburg University Medical Center.

The findings

In the vegan group, methylation patterns shifted at genes involved in immune response, lipid metabolism, insulin signalling and cell growth. At promoters of genes in cancer and growth pathways methylation increased, which tends to mean those genes are being turned down.

For biological age the researchers used three epigenetic clocks. Two of them, trained on health outcomes, showed a slowdown in the vegan group. The third, which estimates chronological age, did not. That split is worth carrying along: of three instruments, two said one thing and one said something else.

The methylation data also allow an estimate of which immune cells were present in the blood and in what proportion. That calculation showed a lower share of neutrophils, which drive inflammation, and a higher share of CD4 T cells, which regulate immune responses.

What the coverage leaves out

Methylation is not the same as gene activity. The study measured marks on the DNA, not how much protein ended up being made. The step from mark to actual effect in the body is plausible, but it was not measured here. When a headline says a vegan diet changes “your genes”, that is a shortcut.

The immune cells were not counted. The proportions of neutrophils and CD4 T cells were estimated from the methylation data. That is an established method, but it remains a prediction. Which is where it gets interesting.

The same Freiburg group did count them, and the result is more restrained. In their own randomised trial over eight weeks with 57 participants, published in BMC Medicine, blood counts were measured directly. There, neutrophils showed only a downward trend that was not significant after adjustment. For CD4 T cells there was no meaningful difference between the groups. Total lymphocytes were in fact lower in the vegan group.

These are two different studies with different participants and different durations, so formally they do not contradict each other. But the immune shift predicted from epigenetics has not yet shown up in the same group's directly measured cell counts. Anyone reading the headline should know that.

What the authors themselves qualify

The researchers put it plainly: the study was small, it ran for only one month, and the changes found were modest and spread across hundreds of thousands of sites. Whether that translates into measurably lower disease risk is open. Larger and longer studies are to follow, and Freiburg University Medical Center has already announced them.

One limitation from the eight-week trial is worth sitting with: because both groups had to hit the same calorie target, the vegan participants also reached for nuts, oils and granola bars. A vegan diet inside a study protocol is therefore not automatically the vegan diet someone assembles voluntarily. What holds up

A clean randomised design, a calorie-matched comparison and a measurable difference after four weeks: that is a solid result, even if it is a small one. The striking part is the speed. That methylation patterns move at all within a month was not a given.

What the study does not show: that vegan people age more slowly, get ill less often or live longer. That needs long observation windows and hard endpoints.

And because nutrition debates tend to fixate on numbers like these: our position does not rest on this study. It would be the same if the methylation patterns had not budged. The reason to eat plants sits at the other end of the chain, with the animals that die for the alternative. Health findings are a welcome side effect, not a foundation.

Humans, not mice: why the method matters too

One detail tends to get lost in nutrition debates: these data come entirely from humans. Blood from 48 volunteers, analysed at more than 800,000 positions in the genome, with no animal model in between. Twenty years ago a question like this would probably have gone to rodents first, in the hope that the result would transfer.

That hope often does not hold.
